Form 10 is one of the application documents for users to be allowed to work at the radiation controlled areas in KEK. The English version of Form 10 was revised recently in accordance with the decision of the Board of Directors, and is scheduled to apply from January 30, 2014,

By this revision, KEK requires more detailed information about your radiation work records as a radiation worker to be certified by the radiation safety officer in your home institute, such as
-personal dose in the previous year
-medical examination and
-training on radiation safety.
