Application of borrowing the PHS

Please submit the application form to the Users Office. The form can be obtained at the Users Office.
A period of use is up to one month. You may use the same PHS on a monthly basis by sumiting an application each month.

If you need to have one for a longer period, please ask the Office of Institute or
　Laboratory at KEK where you belong to.

Calling outside: not available.

Receiving a call from outside: available.
　 How to reach the PHS from outside:
　　029-864-5200 -> message -> XXXX (PHS number: 4 digits)

Duration of lending: in between check-in and check-out date

Returning: Please return the PHS to the Users Office.

Users Office is opening from 9:00 to 17:00 of weekday. 
　Weekend and National Holiday is closed.
　If you can not return during Users Office's opening hour, please return into the
　Phone Return Box at superintendent's office in the Dormitory or the Return Box
　near the Users Office. (Map)

Sublease to the third party is strictly prohibited.

In case you intentionally damage or break the PHS, we will charge you the cost of
　repair or purchasing a new unit.
